Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#74-294-9 with Nadhan, Inc. (dba Creekside Convalescent Hospital and Mental Health Rehabilitation Program), a
corporation, in an amount not to exceed $350,400, to provide Sub-Acute Care for Seriously and Persistently Mentally
Ill Adults, for the period from July 1, 2013 through June 30, 2014.
FISCAL IMPACT:
This Contract is funded 100% by Mental Health Realignment Funds. (Rate Increase)
BACKGROUND:
On January 18, 2011, the Board of Supervisors approved Contract #74-294-6 (as amended by Amendment
Agreements #74-294-7 and #74-294-8) with Nadhan, Inc. (dba Creekside Convalescent Hospital and Mental Health
Rehabilitation Program), for the period from January 1, 2011 through June 30, 2013, for the provision of Sub-Acute
Care for Seriously and Persistently Mentally Ill Adults.
Approval of Contract #74-294-9 will allow Contractor to continue providing Sub-Acute Care for Seriously and
Persistently Mentally Ill Adults through June 30, 2014.
