Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-225-27(7) with Thomas Martin, MFT, a self-employed individual, effective January 1,
2013, to amend Contract #74-225-27(6), to increase the payment limit by $72,000, from $90,000 to a new payment
limit of $162,000, with no change in the original term of July 1, 2012 through June 30, 2014.
FISCAL IMPACT:
This amendment is funded 100% by Medi-Cal funds offset 50% Federal and 50% State. (No rate increase)
BACKGROUND:
In October 2012, the County Administrator approved and the Purchasing Services Manager executed Con-tract
#74-225-27(6) with Thomas Martin, MFT, for the period from July 1, 2012 through June 30, 2014, for the provision
of Medi-Cal specialty mental health services. The utilization during the term of the agreement was higher than
originally anticipated.
Approval of Contract Amendment Agreement #74-225-27(7) will allow the Contractor to provide addition-al
Medi-Cal specialty mental health services through June 30, 2014.

CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, the Contractor will not be able to provide additional Medi-Cal specialty mental
health services. Therefore, patients would have longer wait times and possibly go untreated.
