Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-288-6 with Diablo Nephrology Medical Group, Inc., a corporation, in an amount not to exceed $460,000, to
provide professional nephrology services to Contra Costa Health Plan (CCHP) members for the period from May 1,
2013 through April 30, 2015.
FISCAL IMPACT:
This Contract is funded 100% by member premiums. (No rate increase)
BACKGROUND:
On May 24, 2011, the Board of Supervisors approved Contract #27-288-5 with Diablo Nephrology Medical Group,
Inc., for the period from May 1, 2011 through April 30, 2013 for the provision of professional nephrology services
for the Contra Costa Health Plan.
Approval of Contract #27-288-6 will allow Contractor to continue to provide nephrology services through April 30,
2015.
