HomeMy WebLinkAboutMINUTES - 06042013 - C.62RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract #27-839-1 with Mark N. Isaacs, M.D., Inc., a corporation, in an amount not to exceed $120,000, to provide phlebology services for the Contra Costa Health Plan (CCHP) members for the period from May 1, 2013 through April 30, 2015. FISCAL IMPACT: This Contract is funded 100% by Contra Costa Health Plan member premiums. Costs depend upon utilization. (No rate increase) BACKGROUND: In June 2011, the County Administrator approved and Purchasing Services Manager executed Contract #27-839, with Mark N. Isaacs, M.D., Inc., for the period from May 1, 2011 through April 30, 2013 for the provision of phlebology services. Approval of Contract #27-839-1 will allow Contractor to continue to provide phlebology services through April 30, 2015.
