Approve and authorize the Health Services Director, or his designee to execute on behalf of the County, Contract
#27-253-10 with California Eye Clinic, a partnership, in an amount not to exceed $400,000, to provide professional
ophthalmology and optometry services for the Contra Costa Health Plan for the period from May 1, 2013 through
April 30, 2015.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon
utilization. (No rate increase)
BACKGROUND:
On May 24, 2011, the Board of Supervisors approved Contract #27-253-9 with California Eye Clinic, a partnership,
for the period from May 1, 2011 through April 30, 2013 to provide professional ophthalmology and optometry
services to Contra Costa Health Plan members.
Approval of Contract #27-253-10 will allow the Contractor to continue to provide ophthalmology and optometry
services through April 30, 2015.
