Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-883-21 with Rodney Chan, DPM, a self-employed individual, effective March 1, 2013,
to amend Contract #26-883-19 (as amended by Contract Amendment Agreement #26-883-20), to increase the
payment limit by $8,500, from $293,000 to a new payment limit of $301,500, with no change in the original term of
June 1, 2011 through May 31, 2013.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On May 17, 2011, the Board of Supervisors approved Contract #26-883-19 (as amended by Contract Amendment
Agreement #26-883-20) with Rodney Chan, DPM for the period from June 1, 2011 through May 31, 2013, for the
provision of professional podiatry services for patients at Contra Costa Regional Medical Center and Contra Costa
Health Centers.
