Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#26-924-15 with Theresa Kailikole, DPM, a self-employed individual, in an amount not to exceed $290,000, for the
provision of professional Podiatry services for patients at Contra Costa Regional Medical Center and Contra Costa
Health Centers, for the period from June 1, 2013 through May 31, 2015.
FISCAL IMPACT:
100% Enterprise Fund I. Cost to the County depends upon utilization. As appropriate, patients and/or third party
payor will be billed for services. (No rate increase)
BACKGROUND:
On June 21, 2011, the Board of Supervisors approved Contract #26-924-13 (as amended by Contract Amendment
Agreement #26-924-14) with Theresa Kailikole, DPM, to provide professional

BACKGROUND: (CONT'D)
Podiatry services including, but not limited to, consultation, training, administration and medical and/or surgical
procedures, for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period
from June 1, 2011 through May 31, 2013.
Approval of Contract #26-924-15 will allow the Contractor to continue providing podiatry services at Contra Costa
Regional Medical Center and Contra Costa Health Centers, through May 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, patients requiring these medical services at Contra Costa Regional Medical and
Contra Costa Health Centers would go untreated.
