Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-641-11 with Advanced Medical Personnel Services, Inc., a corporation, effective April
1, 2013, to amend Contract #26-641-10, to increase the payment limit by $200,256, from $550,000 to a new payment
limit of $750,256, with no change in the original term of July 1, 2012 through June 30, 2013.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise I Fund. As appropriate, patients and/or third party payors will be
billed for services. (No rate increase)
BACKGROUND:
On July 31, 2012, the Board of Supervisors approved Contract #26-641-10 with Advanced Medical Personnel
Services, Inc. for the period from July 1, 2012 through June 30, 2013, for the provision of speech, physical and
occupational therapy services to Contra Costa Regional

BACKGROUND: (CONT'D)
Medical Center (CCRMC) and Contra Costa Health Center patients.
Approval of Contract Amendment Agreement #26-641-11 will allow the Contractor to provide additional services
through June 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients requiring these medical services at CCRMC and Contra Costa Health
Centers would experience longer waits and possibly go untreated.
