APPROVE and AUTHORIZE the Director of Behavioral Health Services, or designee, to apply for and accept the
federal Department of Health and Human Services Substance Abuse and Mental Health Services Administration
(SAMHSA) Offender Reentry Program Grant in amount not to exceed $430,000 (each year, for three years), to
expand substance abuse treatment and related recovery and reentry services to sentenced adult offenders returning to
the community from incarceration for criminal offenses.
FISCAL IMPACT:
$1,290,000; federal Department of Health and Human Services SAMHSA Offender Reentry Program. The grant
award may be lower than the amount requested and will be for a period of three years ($430,000 per year) from the
effective date of the award.

BACKGROUND:
The purpose of the SAMHSA Offender Reentry Program (OPR) is to expand and/or enhance substance abuse
treatment and related recovery and reentry services to sentenced adult offenders returning to the community from
incarceration for criminal offenses. Applicants are expected to form stakeholder partnerships that will plan, develop
and provide a transition from incarceration to community-based substance abuse treatment and related reentry
services.
The program should provide offenders with substance use and/or co-occurring mental disorders the opportunity to
improve their lives, including recovery from substance use and mental disorders and developing the capacity and
skills to become parents, employees and citizens in recovery from behavioral health disorders. SAMHSA’s interest is
to actively support and shape offender reentry treatment partnerships so that clinical needs are met and clients are
treated using evidence-based practices consistent with the disease model and the problem-solving model, rather than
with the traditional criminal justice model.
A long-term goal of this program is to build sustainable systems of care for adults needing substance abuse treatment
and recovery support services as they return to the community from incarceration.

Contra Costa County Substance Abuse Treatment and Reentry Program (C-STAR) will serve 50 adult (18-years-old
or older) inmates, each year over three years, identified as “moderate to high risk” with substance use and/or
co-occurring disorders sentenced as adults incarcerated in county jail facilities a minimum of six months to be
released under Probation supervision to Contra Costa County. The County Probation Department will use the
Correctional Assessment and Intervention System (CAIS) to identify “moderate to high risk” inmates for entry into
the program. Participants will be assessed as substance-using/abusing or diagnosed as having a substance use and/or
co-occurring mental disorder by a Substance Abuse Counselor (SAC) working with the Alcohol and Other Drugs
Service Administration (AODS). AODS will coordinate a county-wide collaboration of substance abuse treatment
service providers to serve offenders returning to the county. Pre-release services will be available within four months
of schedule release to the community, and the SAC will provide coordination to ensure enrollment in
community-based treatment upon release.
C-STAR will augment the successes of the AB 109 program. Probation Case Managers (PCM) will conduct an
Actuarial Based Assessment using CAIS and create an Individual Achievement Plan with the participant that includes
developing a transition plan. Cognitive Behavioral Interventions will be provided by PCMs through motivational
interviewing. Health care is provided by Detention Health Services, and the Department of Education will offer
classes in vocational training, GED obtainment, and addressing substance abuse. The Sheriff’s Office will provide
access to Inmate Services, which includes legal assistance, library services, and chaplain services, and oversee inmate
movement within their facilities.
Post-release services: PCM supervision and case management with assistive case management by Substance Abuse
Counselor is offered for the duration of probation. Behavioral Health Services will offer substance abuse treatment,
benefit enrollment, mental health care, including “integrated dual diagnosis-treatment” through the Forensic Team
for participants with co-occurring mental health and substance abuse disorders. Participants will be enrolled for
income and health benefits including Medi-Cal by a Behavioral Health Services SSI Enrollment Specialist. Access to
job placement, retention, and re-placement vocational training will be provided through the County Workforce
Development Board, RichmondWORKS and other community-based providers.
