Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-817-35 with East Bay Nephrology Medical Group, Inc., to amend Contract #26-817-34,
effective March 1, 2013, to increase the total Payment Limit by $24,000 from $156,000 to a new total payment limit
of $180,000 with no change in the original term of the Contract from July 1, 2012 through June 30, 2015.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services. (No increase in rates)
BACKGROUND:
On June 26, 2012, the Board of Supervisors approved Contract #26-817-34 with East Bay Nephrology Medical
Group, Inc., to provide professional nephrology services including consultation, training and/or medical procedures
for patients at Contra Costa Regional Medical Center

and Contra Costa Health Centers, for the period from July 1, 2012 through June 30, 2015.
Approval of Contract Amendment Agreement #26-817-35 will allow the Contractor to provide additional nephrology
services at Contra Costa Regional Medical Center and Contra Costa Health Centers, through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients requiring these medical services at Contra Costa Regional Medical and
Contra Costa Health Centers would have longer wait times and possibly go untreated.
