Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-572-5 with Clark S. Tsai, M.D., a sole proprietor, in an amount not to exceed $200,000, to provide professional
ophthalmology services for the Contra Costa Health Plan for the period from February 1, 2013 through January 31,
2015.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon
utilization. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County. On March 15,
2011, the Board of Supervisors approved Contract #27-572-4 with Clark S. Tsai, M.D., for the period from February
1, 2011 through January 31, 2013, to provide professional ophthalmology services to Contra Costa Health Plan
members. Approval of Contract #27-572-5 will allow the Contractor to continue to provide professional
ophthalmology services through January 31, 2015.
