Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-031-18 with Summitview Child and Family Services, Inc., a non-profit corporation,
effective January 14, 2013, to amend Novation Contract #74-031-17, to increase the payment limit by $105,000,
from $150,000 to a new payment limit of $255,000, with no change in the original term of July 1, 2012 through June
30, 2013, and to increase the automatic extension payment limit by $52,500, from $75,000 to a new payment limit of
$127,500, with no change in the term of the automatic extension, through December 31, 2013.
FISCAL IMPACT:
This amendment is funded 50% by Federal Financial Participation and 50% by Mental Health Realignment. (No rate
increase)
BACKGROUND:
This Contract meets the social needs of County’s population by providing residential day treatment services
including: rehabilitative day treatment, medication support, and other mental health services to, Medi-Cal eligible,
Severely Emotionally Disturbed (SED) children.

BACKGROUND: (CONT'D)
On January 8, 2013, the Board of Supervisors approved Novation Contract #74-031-17 with Summitview Child and
Family Services, Inc. for the period from July 1, 2012 through June 30, 2013, which included a six-month automatic
extension through December 31, 2013, for the provision of residential day treatment and medication support services.
Approval of Contract Amendment Agreement #74-031-18 will allow the Contractor to provide additional services
including an additional level of service, Therapeutic Behavioral Services, through June 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, payment for additional services will not be made and SED children will not have
access to Contractors services and may require higher levels of care.
CHILDREN'S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes: “Children Ready For and
Succeeding in School”; “Families that are Safe, Stable, and Nurturing”; and “Communities that are Safe and Provide
a High Quality of Life for Children and Families”. Expected program outcomes include an increase in positive social
and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS).
