Approve and authorize the Health Services Director, or designee to execute on behalf of the County, Contract
#27-640-5 with Noel T.D. Chiu, M.D., A Medical Corporation, in an amount not to exceed $240,000, to provide
professional dermatology services for the Contra Costa Health Plan for the period from February 1, 2013 through
January 31, 2015.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon
utilization. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County. On March 15,
2011, the Board of Supervisors approved Contract #27-640-4 with Noel T.D. Chiu, M.D., A Medical

BACKGROUND: (CONT'D)
Corporation for the period from February 1, 2011 through January 31, 2013, to provide dermatology services.
Approval of Contract #27-640-5 will allow the Contractor to continue to provide dermatology services through
January 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, certain specialized professional health care services for its members under the terms
of their Individual and Group Health Plan membership contracts with the County will not be provided.
