Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #22-939-16 with Food Bank of Contra Costa and Solano, a non-profit corporation, effective
December 1, 2012, to amend Contract #22-939-14 (as amended by Amendment/Extension Agreement #22-939-15),
to increase the payment limit by $40,606, from $95,000 to a new payment limit of $135,606, with no change in the
original term of March 1, 2012 through February 28, 2013.
FISCAL IMPACT:
This amendment is funded 100% by Ryan White HIV/AIDS Treatment Extension Act. (No rate increase)
BACKGROUND:
On February 17, 2012, the County Administrator approved and the Purchasing Services Manager executed Contract
#22-939-14 (as amended by Amendment/Extension agreement #22-939-15) with Food Bank of Contra Costa and
Solano for the period from March 1, 2012 through February 28, 2013, for the provision of food services for County
residents diagnosed with HIV.

will allow the Contractor to provide an additional level of service, to include nutrition counseling, through February
28, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County residents diagnosed with HIV will not have access to medical nutrition
services authorized under the Ryan White Treatment Act.
