Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Standard
Agreement (Amendment) #29-772-26 (State #04-36067, A-13) with the State of California, Department of Health
Care Services (DHCS), effective January 1, 2013, to amend Agreement #29-772-13 (as amended by subsequent
amendments #29-772-14 through #29-772-25), to incorporate new language and capitation rates for the addition of
Targeted Low-Income Child members with no change in the original term of April 1, 2005 through December 31,
2013 and no change in the original payment limit of $317,472,000.
FISCAL IMPACT:
Approval of this amendment will reflect no change in the original amount payable to County of $317,472,000 for the
Medi-Cal Managed Care Local Initiative Project. No County match required.
BACKGROUND:
On April 26, 2005, the Board of Supervisors approved Standard Agreement #29-772-13 (as amended by subsequent
Amendments #29-772-14 through #29-772-25) with the State of California, DHCS, for the Medi-Cal Local Initiative

BACKGROUND: (CONT'D)
December 31, 2013.
Approval of this Standard Agreement (Amendment) #29-772-26 will incorporate new language and capitation rates
for transition of Targeted Low-Income Child members and to continue providing Medi-Cal managed care services to
qualified County recipients for the Medi-Cal Local Initiative, through December 31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County will not be able receive funds for the transition of Targeted Low-Income
Child members as required by the State of California, Department of Health Services.
