Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #27-697-2 with Prism Services Group, LLC, a limited liability company, effective October
31, 2012, to amend Contract #27-697-1, to increase the payment limit by $80,000, from $80,000 to a new payment
limit of $160,000 and extend the term from October 31, 2012 through October 31, 2014.
FISCAL IMPACT:
This amendment is funded 100% by Contra Costa Health Plan (Health Plan) member premiums in Enterprise Fund
III.
BACKGROUND:
In December 2010 the County Administrator approved and Purchasing Services Manager executed Contract
#27-697-1 with Prism Services Group, LLC, to provide consultation and technical assistance with regard to software
maintenance of an automated system for the Health Plan’s Provider Network for the period from November 1, 2010
through October 31, 2012.
Approval of Contract Amendment Agreement #27-697-2 will allow the Contractor to provide additional services
through October 31, 2014.
