Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #24-680-24 with Telecare Corporation, a corporation, effective June 1, 2012, to amend
Contract #24-680-22, to increase the payment limit by $35,886, from $614,000 to a new payment limit of $649,886,
with no change in the original term of July 1, 2011 through June 30, 2012, and with no change in payment limit or
the term of the automatic extension, through December 31, 2012.
FISCAL IMPACT:
This Contract is funded 100% by Mental Health Realignment. (No rate increase)
BACKGROUND:
On June 21, 2011, the Board of Supervisors approved Contract #24-680-22 with Telecare Corporation for the period
from July 1, 2011 through June 30, 2012, which included a six-month automatic extension through December 31,
2012, for the provision of mental health gero-psychiatric services and subacute care for Severely and Persistently
Mentally Ill patients.

will allow Contractor to be compensated for provision of service to seven additional clients, at two locations, through
June 30, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not be compensated for services authorized by County’s
Behavioral Health Services Division.
