HomeMy WebLinkAboutMINUTES - 12042012 - C.95RECOMMENDATION(S): Approve and authorize the Health Services Director or his designee, to execute, on behalf of the County, Contract #26-710-1 with Janet Diaz, M.D., Inc., a professional corporation, in an amount not to exceed $350,000 to provide professional pulmonary services at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from November 14, 2012 through October 31, 2015. FISCAL IMPACT: This Contract is funded 100% by Enterprise Fund I. Cost to the County depends upon utilization. As appropriate, patients and/or third party payors will be billed for services. (No rate increase) BACKGROUND: On December 13, 2011, the Board of Supervisors approved Contract #26-710 with Janet Diaz, M.D., to provide professional pulmonary services including, consultation, on-call coverage, training, medical procedures at Contra Costa Regional Medical Center and Contra Costa Health Centers, including Richmond and Pittsburg Health Clinics, through November 13, 2012. Approval of Contract #26-710-1 will allow the Contractor to continue to provide services through October 31, 2015.
