Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment/Extension Agreement #27-814-1 with Credentals Services, Inc. (dba Verifpoint/Credentialing
Solutions), a corporation, effective September 30, 2012, to amend Contract #27-814, to increase the payment limit by
$60,000, from $60,000, to a new payment limit of $120,000, and extend the term from September 30, 2012 through
September 30, 2014.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan Member Premiums. (No rate increase)
BACKGROUND:
On November 23, 2010, the Board of Supervisors approved Contract #27-814, with Credentals Services, Inc., for the
period from October 1, 2010 through September 30, 2012, for the provision of credentialing services for healthcare
providers including mutual indemnification to hold harmless both parties for any claims arising out of the
performance of this Contract.

Agreement #27-814-1 will allow the Contractor to continue providing services through September 30, 2014.

If this amendment is not approved, Contractor will not continue providing credentialing services to healthcare
providers.
