Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #27-872-1 with Appian Imaging, LLC (dba Northbay MRI Center), a limited liability
company, effective September 1, 2012, to amend Contract #27-872-1, to increase the payment limit by $350,000,
from $50,000 to a new payment limit of $400,000, with no change in the original term of June 1, 2012 through May
31, 2014.
FISCAL IMPACT:
This amendment is funded 100% by Member Premiums. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County.
In June 2012, the County Administrator approved and the Purchasing Services Manger executed Contract #27-872
with Appian Imaging, LLC (dba Northbay MRI Center) for the period from June 1, 2012 through May 31, 2014, for
the provision of diagnostic imaging services. At the time of negotiations,

the payment limit was based on target levels of utilization; however, utilization of this provider has been higher than
anticipated.
Approval of Contract Amendment Agreement #27-872-1 will allow the Contractor to provide additional diagnostic
imaging services through May 31, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Health Plan members will not have access to the diagnostic imaging services
provided by this Contractor.
