Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-391-20 with Maxim Healthcare Services, Inc., a corporation, effective June 15, 2012, to
amend Contract #26-391-16 (as amended by #26-391-17 and #26-391-18), to increase the payment limit by
$120,000, from $785,000 to a new payment limit of $905,000 with no change in the original term of September 1,
2011 through March 31, 2013.
FISCAL IMPACT:
100% by Enterprise Fund I. No rate increase.
BACKGROUND:
On September 13, 2011, the Board of Supervisors approved Contract #26-391-16 with Maxim Healthcare Services,
Inc. (as amended by Amendment/Extension Agreement #26-391-17 and Administrative Amendment #26-291-18),
for the period from September 1, 2011 through March 31, 2013, for the provision of temporary medical services at
Contra Costa Regional Medical Center and Contra Costa Health Centers.
Approval of Contract Amendment Agreement #26-391-20 will allow the Contractor to provide additional temporary

BACKGROUND: (CONT'D)
medical services for the implementation of the Electronic Health Records EPIC project at Contra Costa Regional
Medical Center and Contra Costa Health Centers, through March 31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contra Costa Regional Medical and Contra Costa Health Centers would not be
able to abstract the desired metadata from medical records for the implementation of the Electronic Health Records
EPIC Project.
