Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-611-6 with Norman B. Livermore, III, M.D., a sole proprietor, in an amount not to exceed $350,000, to provide
professional orthopedic services for the Contra Costa Health Plan for the period from October 1, 2012 through
September 30, 2014.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon
utilization. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County. On September 28,
2010, the Board of Supervisors approved Contract #27-611-5 with Norman B. Livermore, III, M.D., for the period
from October 1, 2010 through September 30, 2012, to provide orthopedic services. Approval of Contract #27-611-6
will allow the Contractor to continue to provide professional orthopedic services through September 30, 2014.
