Approve and authorize the Health Services Director, or his designee, to execute on behalf of the County, Standard
Agreement (Amendment) #29-772-25 (State #04-36067, A-12) with the State of California, Department of Health
Care Services (DHCS), effective October 1, 2012, to amend Agreement #29-772-13 (as amended by subsequent
amendments #29-772-14 through #29-772-24), to incorporate new language and capitation rates for the addition of
Community Based Adult Services (CBAS) with no change in the original term of April 1, 2005 through December
31, 2013 and no change in the original payment limit of $317,472,000.
FISCAL IMPACT:
Approval of this amendment will reflect no change in the original amount payable to County of $317,472,000 for the
Medi-Cal Managed Care Local Initiative Project. No County match required.
BACKGROUND:
On April 26, 2005, the Board of Supervisors approved Standard Agreement #29-772-13 (as amended by subsequent
Amendments #29-772-14 through #29-772-24) with the State of California, DHCS, for the Medi-Cal Local Initiative

BACKGROUND: (CONT'D)
December 31, 2012.
Approval of this Standard Agreement (Amendment) #29-772-25 will allow the County to continue to receive
reimbursement from the State and to continue providing Medi-Cal managed care services to qualified County
recipients for the Medi-Cal Local Initiative, through December 31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County will not be able incorporation new language and capitation rates for the
Community Based Adult Services as required by the State of California, Department of Health Services.
