Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #76-507-12 with Lee Shratter, M.D., a professional corporation, effective September 1,
2012, to amend Contract #76-507-10 (as amended by Amendment Agreement #76-507-11), to increase the payment
limit by $160,600, from $614,000 to a new payment limit of $774,600, with no change in the original term of
January 1, 2011 through December 31, 2013.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. No rate increase.
BACKGROUND:
On January 11, 2011, the Board of Supervisors approved Contract #76-507-10 (as amended by Amendment
Agreement #76-507-11) with Lee Shratter, M.D., to provide radiology services including, but not limited to,
consultation, on-call coverage, supervision and interpretation of CT Scans, MRI, Ultrasounds, invasive procedures
and plain films at Contra Costa Regional Medical

BACKGROUND: (CONT'D)
Center and Contra Costa Health Centers, for the period from January 1, 2011 through December 31, 2013.
Approval of Contract Amendment Agreement #76-507-12 will allow Contractor to provide additional hours of
radiology services to cover during other radiologist absences and continue to providing services, through December
31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients requiring these medical services would have longer wait times and
possibly go untreated at Contra Costa Regional Medical and Contra Costa Health Centers.
