Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-717-1 with Susan Lessin, M.D., a self-employed individual, effective September 1,
2012, to amend Contract #26-717, to increase the payment limit by $115,000, from $125,000 to a new payment limit
of $240,000 with no change in the original term of February 27, 2012 through February 28, 2013.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On March 27, 2012, the Board of Supervisors approved Contract #26-717 with Susan Lessin, M.D., for the provision
of oncology services including clinic coverage, consultation, surgical and/or medical procedures, for the period from
February 27, 2012 through February 28, 2013.
Approval of Contract Amendment Agreement #26-717-1 will allow the Contractor to provide additional oncology
clinic coverage services at Contra Costa Regional Medical Center and Contra Costa Health Centers, through February
28, 2013.

If this amendment is not approved, patients requiring these medical services at Contra Costa Regional Medical Center
