Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #24-837-19 with Daniel May, MD, a self-employed individual, effective August 1, 2012, to
amend Contract #24-837-18, to increase the payment limit by $80,640, from $40,320 to a new payment limit of
$120,960, with no change in the original term of May 1, 2012 through April 30, 2013.
FISCAL IMPACT:
This amendment is funded 67% by Mental Health Realignment and 33% Conditional Release Program. (No rate
increase)
BACKGROUND:
In June 2012, the County Administrator approved and Purchasing Services Manager executed, Contract #24-837-18
with Daniel May, MD for the period from May 1, 2012 through April 30, 2013, for the provision of outpatient
psychiatric services for County’s Conditional Release Program.
Approval of Contract Amendment Agreement #24-837-19 will allow the Contractor to work an additional two days
per week, in order to meet higher than anticipated demand for services, through April 30, 2013.
