Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-400-4 with Rainbow Community Center of Contra Costa County, a non-profit
corporation, effective September 1, 2012, to amend Novation Contract #74-400-3, to increase the payment limit by
$69,000, from $764,000 to a new payment limit of $833,000, with no change in the original term of August 1, 2012
through July 31, 2013, and to increase the automatic extension payment limit by $34,500, from $382,000 to a new
payment limit of $416,500, with no change in the term of the automatic extension, through January 31, 2014.
FISCAL IMPACT:
This Contract is funded 88% by Mental Health Services Act and 12% Federal Financial Participation. (No rate
increase)

BACKGROUND: (CONT'D)
Costa County for the period from August 1, 2012 through July 31, 2013, which included a six-month automatic
extension through January 31, 2014, for the provision of Mental Health Services Act Innovation services.
Approval of Contract Amendment Agreement #74-400-4 will allow the Contractor to provide additional services
through July 31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not be paid for additional services provided to expand the
program to West and East County.
