Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#74-225-5(5) with Laura Sousa, MFT, a self-employed individual, in an amount not to exceed $135,000, to provide
Medi-Cal specialty mental health services, for the period from July 1, 2012 through June 30, 2014.
FISCAL IMPACT:
This Contract is funded 100% by Medi-Cal Funds offset 50% State and 50% Federal. (No rate increase)
BACKGROUND:
On June 22, 2010, the Board of Supervisors approved Contract #74-225-5(4) with Laura Sousa, MFT for the period
from July 1, 2010 through June 30, 2012, to provide Medi-Cal specialty mental health services. Approval of Contract
#74-225-5(5) will allow the Contractor to continue to provide Medi-Cal specialty mental health services through June
30, 2014.
