Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-445-7 with Aaron K. Hayashi, M.D., Inc., a professional corporation, effective June 1,
2012, to amend Contract #26-445-5 (as amended by #26-445-6) to increase the payment limit by $235,000, from
$1,340,000 to a new payment limit of $1,575,000, with no change in the original term of July 1, 2011 through June
30, 2014.
FISCAL IMPACT:
This Amendment is funded 100% by Enterprise Fund I. Increase in Payment Limit is due to additional weeknight and
weekend on-call services. No change in rate of pay.
BACKGROUND:
On June 28, 2011, the Board of Supervisors approved Contract #26-445-5 (as amended by Amendment Agreement
#26-445-6) with Aaron K. Hayashi, M.D., Inc., for the period from July 1, 2011 through June 30, 2014, for the
provision of radiology services including consultation,

BACKGROUND: (CONT'D)
on-call coverage and interpretation of CT Scans, MRIs, Ultrasounds, invasive procedures and plain films for Contra
Costa Regional Medical Center and Contra Cost Health Centers.
Approval of Contract Amendment Agreement #26-445-7 will allow the Contractor to provide additional radiology
weeknight and weekend on-call services at Contra Costa Regional Medical Center and Contra Costa Health Centers,
through June 30, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this Amendment is not approved, patients requiring these medical services at Contra Costa Regional Medical and
Contra Costa Health Centers would have longer wait times and possibly go untreated.
