Approve and authorize the Health Services Director or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #23-498-5 with Sarcom, Inc. (dba the Abreon Group), a corporation, effective June 1, 2012,
to amend Contract #23-498 (as amended by subsequent amendment #23-498-1 through #23-498-4) to modify the
payment provisions to reflect the intent of the parties, with no change to the original payment limit of $224,939 and
no change to the original term of August 15, 2011 through August 15, 2012.
FISCAL IMPACT:
Funding for this amendment is included in the Health Services Department's Enterprise Fund I. This amendment
reflects a change in the payment provisions allocating addition funds to the expenses with no change in the total
Payment Limit.
BACKGROUND:
In August 2011, the County Administrator approved and the Purchasing Services Manager executed Contract
#23-498 (as amended by Amendment Agreements #23-498-1 through #23-498-4) with Sarcom, Inc. (dba the Abreon
Group), for the period from August 15, 2011 through August

BACKGROUND: (CONT'D)
15, 2012, for the provision of consultation and technical assistance to the Department with regard to development of
a strategic communications plan for the Epic Electronic Health Record System.
Approval of Contract Amendment Agreement #23-498-5 allows Contractor to be reimbursed for additional expenses
with regard to the Epic Electronic Health Records Project with no change in the original Payment Limit and no
change in the original term, through August 15, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, the Contractor will not be able to be reimbursed for expenses actually incurred in
the provision of services as requested by the Department with regard to the Epic Electronic Health Records Project.
