Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-642-7 with Chiu-Hung Tung, M.D., Inc., a professional corporation, effective July 1,
2012, to amend Contract #26-642-4 (as amended #26-642-6), to increase the payment limit by $150,000, from
$1,270,000 to a new payment limit of $1,420,000, with no change in the original term of October 1, 2010 through
September 30, 2013.
FISCAL IMPACT:
This Amendment is funded 100% by Enterprise Fund I. No increase in rates except for the addition of a new nine
hour on-call shift.
BACKGROUND:
On September 28, 2010, the Board of Supervisors approved Contract #26-642-4 (as amended by Amendment
Agreement #26-642-6) with Chiu-Hung Tung M.D., Inc., for the period from October 1, 2010 through September 30,
2013, for the provision of anesthesiology services including consultation, training, medical procedures, on-call and
clinical coverage.

BACKGROUND: (CONT'D)
Dr. Tung has consistently been requested by County, and has agreed, to take more on-call shifts for the
Anesthesiology Unit at Contra Costa Regional Medical Center. The original payment limit was based on target levels
of utilization; however, utilization for this physician has been higher than originally anticipated. This Amendment
will address this level of utilization to enable Contractor to continue to provide services, through September 30, 2012,
and also will add a new nine hour on-call shift.
Approval of Contract Amendment Agreement #26-642-7 will allow the Contractor to provide additional
anesthesiology services through September 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, this physician would not be able to be reimbursed for additional anesthesia on-call
services at Contra Costa Regional Medical and Contra Costa Health Centers.
