Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-881-16 with Craig Nielsen, M.D., a self-employed individual, effective July 1, 2012, to
amend Contract #26-881-15, to modify the payment provisions with no change in original payment limit of $450,000
and no change in the original term of October 1, 2010 through September 30, 2013.
FISCAL IMPACT:
This Amendment is funded 100% by Enterprise Fund I. Payment Provisions are being modified to include a nine hour
on-call shift.
BACKGROUND:
On September 28, 2010, the Board of Supervisors approved Contract #26-881-15 with Craig Nielsen, M.D., for the
period from October 1, 2010 through September 30, 2013, for the provision of anesthesiology services including
consultation, training, medical procedures, on-call and clinical coverage.
Approval of Contract Amendment Agreement #26-881-16 will include an additional level of services to allow

the Contractor to provide a nine hour weekday on-call shift, based upon changes in the Operating Room schedule,
and continue to provide anesthesiology services through September 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, this physician would not be able to provide nine hour weekday on-call shifts in the
Operating Room at Contra Costa Regional Medical and Contra Costa Health Centers.
