Approve and Authorize the Health Services Director or his designee, to execute, on behalf of the County, Grant
Agreement #28-831 from the City of Richmond, to pay the County an amount not to exceed $150,000, for the
County’s Community Wellness and Prevention, Richmond Health Equity Partnership (RHEP) Project, for the period
from April 1, 2012 through March 31, 2014.
FISCAL IMPACT:
Approval of this Agreement will result in $75,000 each fiscal year, not to exceed a total of $150,000, from the City of
Richmond for the County’s Community Wellness and Prevention, Richmond Health Equity Partnership Project. No
County match required.
BACKGROUND:
Richmond faces a growing burden of largely preventable chronic illnesses such as heart disease, stroke, obesity, and
diabetes. Public health agencies alone cannot change these environments, but must work collaboratively with the
many other governmental agencies, businesses,

BACKGROUND: (CONT'D)
community-based organizations that are best positioned to create healthy communities. The Richmond Health Equity
Partnership (RHEP) project will develop a Health in all Policies model to promote collaboration between agencies
and integration of health principles into City of Richmond policies. The Health disparities in Richmond are
associated with poor educational outcomes. The goal of the RHEP project will be to improve coordination between
West Contra County Unified School District, the City of Richmond, and Contra Costa County Health Services to
develop a full service community school model to better meet family’s needs, which will in turn increase graduation
rates through March 31, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this agreement is not approved, the County will not receive funds to support and participate in the Richmond
Health Equity Partnership project for in Contra Costa County.
