Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #23-391-7 with Laura Swafford, self-employed individual, effective February 1, 2012, to
amend Contract #23-391-6, to increase the payment limit by $70,000, from $75,000 to a new payment limit of
$145,000, with no change in the original term of July 1, 2011 through June 30, 2012.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise I Budget. (No rate increase)
BACKGROUND:
In August 2011, County Administrator Approved and Purchasing Services Manager executed Contract #23-391-6
with Laura Swafford for the period from July 1, 2011 through June 30, 2012, for the provision of consultation and
technical assistance to the Department with regard to development of a strategic communications plan for the Epic
Electronic Health Record System.

BACKGROUND: (CONT'D)
#23-391-7 will allow the Contractor to provide additional hours of consultation and technical assistance to the
Department with regard to Keane Patcom System and the Epic Electronic Health Records Project, through June 30,
2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, the Contractor will not be paid for additional hours of services and the
Department will not benefit from the Contractor’s expertise.
