Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-317-6 with Alternative Family Services, Inc., a non-profit corporation, effective May 1,
2012, to amend Novation Contract #74-317-5, to increase the payment limit by $100,000, from $500,000 to a new
payment limit of $600,000, with no change in the original term of July 1, 2011 through June 30, 2012, and to
increase the automatic extension payment limit by $50,000, from $250,000 to a new payment limit of $300,000, with
no change in the term of the automatic extension, through December 31, 2012.
FISCAL IMPACT:
This Amendment is funded by 50% by Federal Financial Participation and 50% by State Early and Periodic
Screening, Diagnosis, and Treatment (EPSDT). (No rate increase)
BACKGROUND:
This Contract meets the social needs of County’s population in that it provides Multidimensional Treatment Foster
Care (MTFC) services to Seriously Emotionally Disturbed

BACKGROUND: (CONT'D)
(SED) youth who are in foster care or in Intensive Treatment Foster Care homes, and their families, including mental
health services, medication support, and crisis intervention.
On November 15, 2011, the Board of Supervisors approved Novation Contract #74-317-5 with Alternative Family
Services, Inc. for the period from July 1, 2011 through June 30, 2012, which included a six-month automatic
extension through December 31, 2012, for the provision of MTFC services to SED adolescents.
Approval of Contract Amendment Agreement #74-317-6 will allow the Contractor to provide additional services
through June 30, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, additional multidimensional treatment foster care services for seriously
emotionally disturbed youth and their families will not be provided.
CHILDREN'S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes: “Children Ready For and
Succeeding in School”; “Families that are Safe, Stable, and Nurturing”; and “Communities that are Safe and Provide
a High Quality of Life for Children and Families”. Expected program outcomes include an increase in positive social
and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS).
