Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#72-038 with Nurse-Family Partnership, a Non-Profit Corporation, in an amount not to exceed $140,000, to
implement a Nurse-Family Partnership Program to provide home visits for first time mothers in Contra Costa County,
for the period from June 1, 2012 through June 30, 2015.
FISCAL IMPACT:
This Contract is funded 100% by the California Department of Public Health Grant.
BACKGROUND:
The California Department of Public Health, Maternal, Child and Adolescent Health (CDPH/MCAH) Division,
California Home Visiting Program has awarded recipients of Maternal, Infant and Early Childhood Home Visiting
funding to implement a Nurse-Family Partnership (NFP) within at-risk communities in California. The Nurse-Family
Partnership Project represents a highly refined approach to the long-established service strategy of home visiting.

BACKGROUND: (CONT'D)

pregnancy and the first two years of the child’s life.
Approval of Contract #72-038 will allow Contractor to implement a Nurse-Family Partnership Program for first time
mothers in Contra Costa County, through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, Contra Costa County first time mothers would not receive the services of the
Nurse-Family Partnership Program.
CHILDREN'S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes: “Families that are Safe, Stable,
and Nurturing”; and “Communities that are Safe and Provide a High Quality of Life for Children and Families”.
Expected program outcomes include better pregnancy outcomes, improve the child’s health and development and
help the families become economically self-sufficient by working with parents to develop a vision of their own
future, plan future pregnancies, continue their education and find jobs.
