HomeMy WebLinkAboutMINUTES - 06262012 - C.97RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #22–970–7 with Center for Human Development, a non-profit corporation, effective April 1, 2012 to amend Contract #22–970–6, to increase the payment limit by $40,000, from $329,638 to a new payment limit of $369,638, with no change in the original term of July 1, 2011 through June 30, 2012. FISCAL IMPACT: This Contract is funded by 5% Medi-Cal Administration Activities, 87% Enterprise I Funds and 8% Kaiser Foundation Grant. (No rate increase) BACKGROUND: On August 2, 2011, the Board of Supervisors approved Contract #22-970-6 with Center for Human Development for the period from July 1, 2011 through June 30, 2012, for the provision of health education and outreach services to Spanish speaking residents of Contra Costa County. Approval of Contract Amendment Agreement #22-970-7 will allow the Contractor to provide additional services through June 30, 2012. Expected program outcomes include an increase in nutritious food and physician activity.
