Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-977-12 with Diablo Valley Oncology and Hematology Medical Group, a corporation,
effective January 1, 2012, to amend Contract #26-977-9 (as amended by #26-977-10 and #26-977-11), to modify
payment provision with no change in original Payment Limit of $300,000 and with no change in term of November 1,
2009 through October 31, 2012.
FISCAL IMPACT:
This Amendment is funded 100% by Enterprise Fund I and will reflect an additional rate of $300 per report for peer
review reports requested by County.
BACKGROUND:
On November 3, 2009, the Board of Supervisors approved Contract #26-977-9 (as amended by Amendment
Agreement #26-977-10) with Pacific Urology for the period from November 1, 2009 through October 31, 2012, for
the provision of urology services to patients at Contra Costa

BACKGROUND: (CONT'D)
Regional Medical Center and Contra Costa Health Centers (CCRMC) including, but not limited to, clinic coverage,
consultation, training, on-call, ultra sound, evaluation and management services, and medical and/or surgical
procedures. In April 2012, Assignment #26-977-11 County and Pacific Urology agreed to the assignment of the
Contract from Pacific Urology to Diablo Valley Oncology and Hematology Medical Group.
Approval of Contract Amendment Agreement #26-977-12 will allow the Contractor to provide an additional level of
services, including performing peer review reports at CCRMC, through October 31, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this Amendment is not approved, the Contractor would not be able to perform peer review reports for the Urology
Department at Contra Costa Regional Medical Center and Contra Costa Health Centers.
