Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-539-2 with RecordXpress of California, LLC, a limited liability company, effective June
1, 2012, to amend Contract #26-539 (as amended by #26-539-1), to allow Contractor to provide additional medical
records storage, management and retrieval services, with no change in the original payment limit of $2,905,000 and
no change in the original term of August 1, 2005 through July 31, 2013.
FISCAL IMPACT:
This Amendment is funded 100% by Enterprise Fund I. The revised rate sheet reflects an increase in rates.
BACKGROUND:
On August 16, 2005, the Board of Supervisors approved Contract #26-539 (as amended by Amendment Agreement
#26-539-1) with RecordXpress of California, LLC, for the period from August 1, 2005 through July 31, 2013, to
provide off-site storage of records, including 24 hours per day/ 7 days per week pick-up and retrieval services,
management of the process of storage, retrieval,

BACKGROUND: (CONT'D)
and re-file of all hard-copy and radiological records, as well as secured destruction of records held in storage
(primarily patient medical records), for Contra Costa Regional Medical Center and Contra Costa Health Centers, and
other designated Health Services Department Divisions.
Approval of Contract Amendment Agreement #26-539-2 will allow the Contractor to provide additional record
storage and transportation for approximately 1,000 to 2,000 medical records per day between Martinez and eight (8)
clinic locations, through July 31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this Amendment is not approved, the Health Services Department would not have record storage and transporation
for approximately 1,000 to 2,000 additional medical records.
