Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#23-312-8 with La Clinica de la Raza, Inc., a non-profit corporation, in an amount not to exceed $150,000, to provide
primary medical care services for low income uninsured residents of Contra Costa County, for the period from May
1, 2012 through April 30, 2013.
FISCAL IMPACT:
This Contract is funded 100% from Low Income Health Plan funds. (No rate increase)
BACKGROUND:
On May 5, 2009, the Board of Supervisors approved Contract #23-312-5 (as amended by Contract Extension
#23-312-6 and Amendment Agreement #23-312-7) with La Clinica de la Raza, Inc., to provide primary medical care
services to low income uninsured residents of Contra Costa County for the period from May 1, 2009 through April
30, 2012.
Approval of this Contract #23-312-8, will allow contractor to continue

BACKGROUND: (CONT'D)
to provide primary medical care services to low income uninsured residents of Contra Costa County, through April
30, 2013. The General Conditions were modified from the standard County format.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, patients will experience longer wait times and potentially go untreated, due to
reduced access to the medical services provided at Contractor’s clinic.
