Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Standard
Agreement (Amendment) #29-784-5 (State #03-75796, A07) with the State of California, Department of Health Care
Services, effective December 30, 2012, to amend Standard Agreement #29-784 (as amended by Amendment
Agreements #29-784-1 through #29-784-4), to extend the term from December 31, 2012 through December 31,
2013, with no change in the original payment limit of $1,594,000.
FISCAL IMPACT:
Approval of this amendment will reflect no change in the original amount payment to County of $1,594,000 for the
Local Initiative Program services that are not approved for Federal funding. No County match required.
BACKGROUND:
Since 2003, through the Local Initiative Program, the State of California has given the Health Plan funds necessary to
provide health care service to its eligible Medi-Cal recipients within the scope of Medi-Cal benefits. These services
were formerly provided under Standard

BACKGROUND: (CONT'D)
Agreement #29-772 and are now being broken out under this new Agreement Amendment #29-784. On August 15,
2006, the Board of Supervisors approved Standard Agreement Amendment #29-784 (as amended by Amendment
Agreements #29-784-1 through #29-784-4) with the California DHCS for the period from August 1, 2003 through
December 31, 2012.
Approval of Standard Agreement (Amendment) #29-784-5 will allow the County to continue to receive
reimbursement from the State and to continue providing the related services to Contra Costa Health Plan members
and County recipients, through December 31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, funding for continuous services to County Medi-Cal recipients will not be
provided.
