HomeMy WebLinkAboutMINUTES - 05082012 - C.73RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #26-445-6 with Aaron K. Hayashi, M.D., Inc., a professional corporation, effective April 1, 2012, to amend Contract #26-445-5 to increase the payment limit by $95,000, from $1,245,000 to a new payment limit of $1,340,000, with no change in the original term of July 1, 2011 through June 30, 2014. FISCAL IMPACT: This Contract is funded 100% by Enterprise Fund I. Increase in Payment Limit is due to additional weeknight and weekend on-call services. No change in rate of pay. BACKGROUND: On June 28, 2011, the Board of Supervisors approved Contract #26-445-5 with Aaron K. Approval of Contract Amendment Agreement #26-445-6 will allow the Contractor to provide additional weeknight and weekend on-call services at Contra Costa Regional Medical Center and Contra Costa Health Centers, through June 30, 2014. CONSEQUENCE OF NEGATIVE ACTION: If this Amendment is not approved, the Contractor will not be able to provide additional radiology on-call services at Contra Costa Regional Medical Center and Contra Costa Health Centers, as requested by County.
