Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-610-5 with Michael David Kassels, DO, a self-employed individual, in an amount not to exceed $200,000, to
provide professional primary care services for the Contra Costa Health Plan for the period from June 1, 2012 through
May 31, 2013.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon
utilization. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County. Under Contract
#27-610-5, the Contractor will provide professional primary care services to Contra Costa Health Plan members,
through May 31, 2013.
