Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #76-524-9 with Samir B. Shah, M.D., Inc.,
a professional corporation, effective March 1, 2012, to amend Contract #76-524-7 (as
amended by #76-524-8) to increase the payment limit by $65,000, from $1,459,100 to a
new payment limit of $1,524,100, with no change in the original term of June 1, 2010
through May 31, 2013.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increases)
BACKGROUND:
On June 8, 2010, the Board of Supervisors approved Contract #76-524-7 (as amended by
Amendment Agreement #76-524-8) with Samir B. Shah, M.D., Inc., for the period from
June 1, 2010 through May 31, 2013, for the provision of ophthalmology services at Contra
Costa Regional Medical Center and Health Centers including, but not limited to,
consultation, clinic coverage, training,

BACKGROUND: (CONT'D)
on-call services and medical and/or surgical providers.
At the time of negotiations the Payment Limit was based on target levels of utilization.
However, ophthalmology clinic coverage by Dr. Shah was higher than originally
anticipated. Approval of Contract Amendment Agreement #76-524-9 will allow the
Contractor to continue to provide ophthalmology services at Contra Costa Regional Medical
Center and Health Centers, through May 31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Dr. Shah would not be able to provide necessary
ophthalmology clinic coverage, requested by the County, through the term of the contract.
