HomeMy WebLinkAboutMINUTES - 04172012 - C.63RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #26-573-2 with Dominic Ciotola (dba CPR Training Center), a sole proprietor, effective March 1, 2012, to amend Contract #26-573-1, to increase the payment limit by $25,000, from $160,000 to a new payment limit of $185,000, with no change in the original term of July 1, 2009 through June 30, 2012. FISCAL IMPACT: This amendment is funded 100% by Enterprise I. (No rate increase) BACKGROUND: On July 7, 2009, the Board of Supervisors approved Contract #26-573-1 with Dominic Ciotola (dba CPR Training Center) for the period from July 1, 2009 through June 30, 2012, for the provision of American Heart Association-approved Cardiopulmonary Resuscitation (CPR) course including, but not limited to, Basic Life Support, Advanced Cardiac Life Support (ACLS) and Advanced Pediatric Cardiac Life Support (APCLS) Courses for Contra Costa Regional Medical Center (CCRMC).
