Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #22-986-4 with Monument Community
Partnership, a non-profit corporation, effective March 1, 2012, to amend Contract
#22-986-3, to increase the payment limit by $112,444, from $121,197 to a new payment
limit of $233,641, with no change in the original term of July 1, 2011 through June 30,
2012.
FISCAL IMPACT:
This amendment is funded 67% by California Nutrition Network, 11% Caltrans, and 22%
Community Wellness and Prevention Program. (No rate increase)
BACKGROUND:
This Contract meets the social needs of County’s population by ensuring County and its
providers are meeting the requirements of the United States Department of Agriculture
(USDA) and the California Nutrition Network.
On June 7, 2011, the Board of Supervisors approved Contract #22-986-3 with Monument

BACKGROUND: (CONT'D)
of consultation and technical assistance to the Department’s Community Wellness and
Prevention Program (CW&PP) with regard to program compliance.
Approval of Contract Amendment Agreement #22-986-4 will allow the Contractor to
provide additional services through June 30, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County will not have access to Contractor’s expertise in
order to determine whether the CW&PP nutrition program, and its subcontractors, is in full
compliance with USDA and State nutrition guidelines for allowable expenses.
