Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract #27-610-5 with Michael David Kassels, DO, a self-employed
individual, in an amount not to exceed $200,000, to provide professional primary care
services for the Contra Costa Health Plan for the period from March 1, 2012 through
February 28, 2013.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member
premiums. Costs depend upon utilization. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care
services for its members under the terms of their Individual and Group Health Plan
membership contracts with the County. In July 2011, the County Administrator approved
and the Purchasing Services Manager executed Contract #27-610-4 with Michael David
Kassels, DO, for the period from March 1, 2011 through February 29, 2012, to provide
primary care services. Approval of Contract #27-610-5 will allow the Contractor to continue
