Approve and authorize, the Health Services Director, or his designee, to execute, Grant
Agreement #28-830 with Regents of the University of California, San Francisco, to pay the
County an amount not to exceed $89,000 for the County Public Health Division’s Reducing
Infant Mortality Disparities by Addressing Social Determinants of Health project, for the
period from September 1, 2011 through August 31, 2012.
FISCAL IMPACT:
Approval of this grant agreement will result in an amount not to exceed $89,000 for the
reducing infant mortality disparities project through August 31, 2012. (No County match
required).
BACKGROUND:
The “Reducing Infant Mortality Disparities” by addressing social determinants of health
project is collaboration between the Regents of California, Contra Costa County,
community-based organizations, and County residents participating in the Black Infant
Health Program. The goal of the project is to increase individual and community-level
awareness of disparities

BACKGROUND: (CONT'D)
affecting African American birth outcomes and infant mortality; promote critical reflection
and dialogue among women and community members about the social factors that
influence health related behaviors and outcomes; and increase women’s self-efficacy and
competence in advocating for themselves their families and their communities.
Approval of Grant Agreement #28-830 will allow funding to support the County’s Public
Health’s reducing infant mortality disparities by addressing social determinants of health
project, including mutual indemnification to hold the other party harmless from any claims
arising out of the performance of this Contract.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, the County will not receive funding to support the Public
Health’s reducing infant mortality disparities project.
