Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #22-806-19 with Public Health Foundation
Enterprises, Inc., a non-profit corporation, effective February 1, 2012, to amend Contract
#22-806-18, to increase the payment limit by $150,000, from $803,754 to a new payment
limit of $953,754, with no change in the original term of July 1, 2011 through June 30,
2012.
FISCAL IMPACT:
This amendment is funded 38% by Federal Bioterrorism Grant, 11% by California Tobacco
Grant, 20% by Health Care Interpreter Network Reimbursement, and 31% by budgeted
County funds.
BACKGROUND:
On June 7, 2011, the Board of Supervisors approved Contract #22-806-18 with Public
Health Foundation Enterprises, Inc., for the period from July 1, 2011 through June 30, 2012,
for the provision of consultation and technical assistance with regard to community health
promotion for the Partnership for Public Health Program.

Approval of Contract Amendment Agreement #22-806-19 will allow the Contractor to
provide additional services through June 30, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County residents who depend on services from West
County Health Collaborative, Monument Community Partnership, and Bay Point
Community Partnership Programs may not receive the services they need.
