HomeMy WebLinkAboutMINUTES - 03132012 - C.58RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #74-294-7 with Nadhan, Inc. dba Creekside Convalescent Hospital and Mental Health Rehabilitation Program, a corporation, effective January 1, 2012, to amend Contract #74-294-6, to increase the payment limit by $306,600, from $306,600 to a new payment limit of $613,200, with no change in the original term of January 1, 2011 through December 31, 2012. FISCAL IMPACT: This amendment is funded 100% by Mental Health Realignment. Approval of Contract Amendment #74-294-7 will allow the Contractor to provide additional services through December 31, 2012. CONSEQUENCE OF NEGATIVE ACTION: If this amendment is not approved, seriously and persistently mentally ill adults will not receive sub-acute care for services. CHILDREN'S IMPACT STATEMENT: Not Applicable
